Fall Risk Management

When performing our initial assessment, we take the time to go over in-home safety.


We may make suggestions on fall prevention equipment such as walkers, raised toilet seats, grab bars, use of a transfer belt (gait belt) or suggest removing trip hazards such as unsecured area rugs. In addition, if you or your loved one is under the care of a physical therapist and/or occupational therapist, we will follow the lead and recommendations of these healthcare professionals to help prevent falls and ensure you or your loved one’s safety.


Our caregiver teams are notified ahead of time if you or your loved one is at risk for falls and what techniques and/or equipment have been implemented to help prevent further falls. If there is a decline in your loved-one’s or your condition, we will come out and reassess.
